It draws a 1.8–2.5mm line, broad enough to fill and fine enough to draw with, and it comes in the widest colour range of any POSCA. If you are only buying one, this is the one.
The 5M line is 1.8–2.5mm against the 3M at 0.9–1.3mm. The 5M covers ground faster and has the most colours; the 3M is better for detail and small work. Many artists own both.
Yes — the 3M and 5M are the usual choices for fabric. Slip card inside so the paint does not transfer through, let it dry completely, then heat-set: garment inside out, a cloth between the iron and the fabric, dry iron with no steam for a couple of minutes. Sneakers flex a lot, so seal them with an acrylic finisher as well.
Yes. The PC-5M nib is reversible, bullet at one end and chisel at the other, and a worn nib pulls out with tweezers so a replacement goes straight in.
